Rehabilitation TeamIn our newly renovated & spacious rehab room, patients undergo intense rehabilitation & gradual reconditioning for a host of conditions including:

Our Rehabilitative Team optimizes Short-Term Rehab Therapies up to 6 days a week using state-of-the-art equipment in:

Short-Term Rehab / Specialized Orthopedic

Succesfully returning our patients to the community.
Our patients are fully engaged in their rehabilitative process, playing a central role in their plan of care from admission through discharge.

Aggressive therapy is provided daily, including weekends, for as many hours as needed. As our patients progress and make strides in their strength, endurance and mobility – we encourage them to get back to their lives and loved ones at the earliest possible moment.

Our care plans are tailor-made for each patient as they strive to attain their highest level of independence. Responding to the particular needs of the younger, more active patient, who is intent on returning to work and family as soon as possible, as well as the special concerns of the geriatric patient – our Rehab program is noted for its keen sensitivity.

Long-Term Skilled Care

We know how dear your loved one is to you.
At Long Island Care Center – our highly specialized spheres of excellence revolve around our patients’ and residents’ individual needs in a home-like environment.

Our nursing staff are all caring individuals with a heightened sensitivity to the needs of our patients. They exude a real sense of warmth and compassion. Our different specialty units are staffed by nurses specially trained in that particular discipline, who implement the targeted medical care recommended by their attending physicians.

Our nurses are on the front lines as they play a pivotal role in helping our patients to heal physically and emotionally while they achieve their maximum goals.

Our Wound Care Physician and specialized wound care team conduct weekly rounds (at minimum) to assess, treat and heal all complex and non-complex wounds.

Subacute Care

Our interdisciplinary approach to medical & nursing care is key.
Our dedicated team of physicians, specialists and nurses work together to ensure an unparalleled level of day-to-day hands-on care. In coordination, they frequently review every patient’s course of treatment and make modifications based on their changing needs.

Our nurses are on the front lines as they play a pivotal role in helping our patients to heal physically and emotionally while they achieve their maximum goals.

Our Wound Care Physician and specialized wound care team conduct weekly rounds (at minimum) to assess, treat and heal all complex and non-complex wounds.

Our patients’ comfort & care are of utmost importance.

Our 6th Floor Subacute and Room come equiped with 32" Plasma TV's, cable and phone services available.

Vent Unit / Respiratory Care

Every breath we take is a gift;
at Long Island Care we don’t take that for granted.
Having your loved one on a ventilator is an emotionally stressful experience; we view this as a transitional stage. Our round-the-clock team of Respiratory Therapists are here to provide and ensure the utmost of care and maximum results.

Rehabilitation Program StaffOur major objective is to wean our patients off the ventilator, provide them with rehabilitation therapies and to return them to their optimum level of health. That level is attained when patients are breathing on their own once again and have reached their highest level of function.

We look forward to returning them back to their homes to lead normal lives.

Our ventilator unit is staffed by an expert team of specialists. A dedicated RN and Respiratory Therapist are on hand 24/7, and a Pulmonologist oversees the care and weaning process of our residents.

Our NY State Certified Ventilator unit consists of state-of-the-art machinery & the latest technology – the very same equipment found in most hospital intensive care units.

Our Respiratory Therapist, Speech Pathologist and Registered Dietician work together to return our vent patients to a normal diet whenever possible.

We provide specialized recreational activities to stimulate our vent patients on a one-to-one basis.

Hospice Unit

Our Hospice Unit is dedicated to helping ease the transition
for the patient and family during this most difficult time.
Our dedicated unit consists of five large beautifully appointed Private Rooms, with available accommodations for overnight stays with your loved ones as well as 24 hour family visits.

Long Island Care Center provides strong family support throughout this painful period.

Our highly trained staff is constantly focused on easing the pain and suffering of our hospice patients. Pain Management plays a very important role in ensuring the comfort of our patients.

Asian Unit

Asian UnitOur 4th floor is dedicated to all things Asian, making our Asian residents and their families feel completely at home while on the road to recovery.

Our staff is fluent is most Asian languages, and the aroma of Asian food wafts through the halls. Movies from Thailand, Malaysia, Korea and China are shown regularly, and origami and Mahjong are common recreational activities.

Traditional Chinese food is served daily.
